The Gordon Bowen Family Assistance Fund (FAF) was established to allow us to offer further reductions in the cost of housing for eligible guests. Supported by charitable giving, the FAF is not an unlimited fund. The use of FAF dollars is determined by established criteria, including: a family’s financial need, their fundraising activity, availability of other resources, length of stay, and personal circumstances.
Prior to filing this form, you will want to gather information about options for financial support.
- Ask your Transplant Team Social Worker if you are eligible to receive lodging assistance from the hospital.
- If you have private insurance, contact the Customer Service number on your insurance company card and ask if you or your patient are eligible for lodging or travel benefits.
If so, some of the questions you should ask include:
- Is there a daily limit?
- How much is your annual and/or lifetime benefit?
- Are both patient and caregiver covered at the same level?
- Are there any special circumstances?
- Is there a miles-from-home requirement?
- Are there restrictions depending upon stage of treatment
